About the Candidate

Biographical Sketch

Please provide a brief biographical sketch which describes the background and accomplishments of the candidate and how they meet the selection criteria of this award. The biographical sketch should be 250-300 words in length.

Letters of Reference

The first nominator's nomination letter should be 1-2 page in length. Please also include two (2) additional one-page letters of reference that outline why the candidate deserves the Lifetime Achievement Award.

At least one letter must be from a person with expertise in the field of Physical Education, Health Education and/or Healthy Schools.
Please be sure to share the nature of their affiliation with their governing bodies that show a long standing impact, i.e. for how long and in which roles, in the letters of reference.
All letters must be signed by the respective nominator.
